INSURANCE INFORMATION
As a courtesy service to you, we are happy to submit your claim to your dental insurance carrier. We ask that you provide us with all of your insurance information at the time you schedule your appointment. We will then contact your insurance carrier to find out your specific plan’s insurance benefits, and let you know your estimated portion prior to your appointment.
Be advised insurance companies routinely indicate that coverage verification does not guarantee payment. While we try our best to estimate your out-of-pocket expense, actual payment is determined by the insurance company once they process the claim.
At the time of service, you will be responsible for the estimated portion that your insurance will not cover. Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage. We will wait 90 days for the insurance company to pay their portion. In the event your insurance company fails to pay, rejects the claim, or does not pay the total balance, you are responsible for the remaining balance.
If your insurance pays more than the estimated amount, a refund check from this office will be mailed within 1 month from the date payment is received in our office.
If your insurance pays less than the estimated amount, you will receive a statement from this office. Your prompt remittance is greatly appreciated.
Currently, we are contracted providers for Delta Dental, CIgna HMO, Aetna, and Four-star discount plan.
While we may not be in contract with your particular dental plan, it does not mean your insurance company completely excludes our office. Many insurance companies pay out of network dentists equal to their in network dentists. Our staff will research your insurance and give you the best out of pocket estimate prior to your visit.
